• If you should warrant immediate medical attention during the in person event, you grant permission to the medical personnel selected by the instructor to review your records or contact the appropriate physician, psychiatrist, health professional, or psychologist to obtain additional information on the conditions noted. 
• In person events, If you should warrant immediate medical attention at the event, you hereby grant permission to the physician selected by Helen Craven and representatives to advocate.
   

  ASSUMPTION OF RISKS

  I acknowledge that I have voluntarily applied to participate in the event designated above.  I am voluntarily participating in the event with the knowledge that travel involves numerous risks and dangers, including, but not limited to, a risk of illness, injury, or death, which may be caused by forces of nature, civil unrest, terrorism, road conditions, injuries inflicted by animals, insects, reptiles or plants, trails, hotels, vehicles, boats or other means of conveyance; high altitude accident, accident or illness without access to means of rapid evacuation or the availability of medical supplies; and the lack of adequacy of medical attention once provided.  I assume the risk of negligence on the part of Helen Craven, their affiliated organizations, and their employees, teachers, volunteers, or other agencies in connection with said event. 
  I HEREBY ACCEPT ANY AND ALL RISKS OF ILLNESS, INJURY, EMOTIONAL TRAUMA, OR DEATH AND VERIFY THIS STATEMENT BY CLICKING YES TO THE TERMS & CONDITIONS
  CANCELLATION POLICY
  There is no refunds for cancelling. If, and only if the instructor cancels the event you will receive full refund.
   
  Helen Craven, reserves the right to cancel any event before the date of the event for any reason whatsoever, including an insufficient number of participants or logistical problems (e.g. acts of God, war, strikes, etc.), which may impede event operations.  The refund of all payments as described in the paragraph above shall release Helen Craven, from any further liability.  An event cancellation due to insufficient numbers would normally occur at least a week before date, unless acts of God, war, strikes or other events impede our departure, causing large numbers of participants opt to cancel participation on the journey in the last days before departure. 
  KNOWING AND VOLUNTARY EXECUTION
  I have carefully read and fully understand the contents of this agreement.  I am aware, that this is a release of liability and a legally binding and enforceable contract between myself and Helen Craven and I respond of my own free will.
  By clicking Yes to this agreement, you grant permission to Helen Craven to capture photos and videos of you during the agreed-upon events or activities. You acknowledge that these images may be used in advertising materials, promotional content, or other marketing purposes at the discretion of Kathy Milanowski. You waive any right to inspect or approve the finished products that may be used in connection with such materials.
   
  RELEASE AS LAWFUL CONSIDERATION for being permitted by Helen Craven, to participate in such event and accompanying activities, I hereby agree that I, my heirs, legal representatives or any member of my family will not make a claim against or sue Helen Craven, or any of their affiliated agents, employees, teachers or volunteers, for bodily injury, death, emotional trauma or property damage resulting from my participation in the event.  
  Helen Craven shall not be liable, directly or indirectly, for bodily injury or property damage for any reason whatsoever, including, but not limited to, the following causes: acts of God, pandemics, animals, epidemics, detention, annoyance, weather, quarantines, strikes, civil disturbance, theft, negligence including negligence of Helen Craven, default, changes in government regulations, terrorism, war or failure of conveyance to arrive or depart as scheduled.
  I therefore release and discharge Helen Craven and their agents, employees, teachers, and volunteers from and against any and all liability arising from my participation in the event, even though that liability may arise out of negligence or carelessness on the part of Helen Craven,  or their affiliate agents, employees, teachers or volunteers.
  If any portion of this agreement is unenforceable, the remaining portions shall remain in full force and effect.  Under no circumstances shall Helen Craven,  be liable to any event participant for more than the land cost of the event applied for on this document. I waive any charge-back rights and in the event of a dispute, requests for a refund will be made using the procedures described under the “Cancellation and Refunds” section below. All applications are subject to acceptance by Helen Craven. Upon acceptance of my application, this agreement shall be deemed to have been entered into at Yeaveley, Derbyshire, and shall be construed according to the laws of the UK.  In the unlikely event a legal dispute should arise involving any subject matter whatsoever, I agree that the exclusive venue for any legal action shall be Yeaveley, Derbyshire, UK. 
• I understand that if I suspect I may have a medical condition I should seek help from a qualified medical
  practitioner.
• I have been advised that if I take any prescription drugs, I must first consult my GP/consultant before
  making any changes.
• I understand that all information will be treated in strictest confidence.
• I confirm that the details given by me to the Practitioner are correct and that if any of the personal
  information given changes, then I accept that I must inform the Practitioner accordingly.

• I understand that Reiki is a simple, gentle, energy used in healing. I understand that Reiki practitioners do
  not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor
  interfere with the treatment of a licensed medical professional. Reiki does not take place of medical care.
• Reiki is not intended to replace or act as an alternative to conventional treatments given by a vet. The
  Veterinary Surgery Act of 1966 (amended 1996) prohibits anyone other than a Veterinary Surgeon to
  diagnose ailments and giving of advice on such diagnosis. The Protection of Animals Act 1911 requires
  that if an animal clearly needs treatment from a Veterinary Surgeon then the owner must obtain this.
• Veterinary permission is no longer required before treating your horse, it is now the responsibility of the
  owner to ensure that the therapist is fully qualified and insured. I reccomend that you contact your vet if
  you have any health concerns abot your horse.
• The healing of animals by contact healing, by the laying on of hands or distant healing is legal. To give
  emergency first aid to animals for the purpose of saving life or relieving pain is permissable under the
  Veterinary Surgeons Act 1996.
• I have been advised that if I have a medical condition, I should seek help from a qualified medical
  practitioner or veterinarian.
• I confirm that the details given by me to the Practitioner are correct and that if any personal
  information changes, then I accept that I must inform the Practitioner accordingly.
